# Patient documents (/docs/en/guides/patient-documents)

**Documents & files** in a patient's record lists the documents you write, the visit notes and the files you upload. Writing a document needs write access to the patient's clinical records.

## Create or find a document [#create]

1. Open the patient and choose **New → Document**. A blank document opens.
2. Give it a title and write the body. It saves as you type.
3. Check that the indicator reads **Saved** before you close the tab. **Saved** means the text reached Salisan. It does not mean anything was recorded, signed or sent.
4. To find it later, open **Documents & files**, search with **Search documents** and use the chips **All**, **Documents**, **Files** and **Visits**.

Visit notes appear in the same list, marked **Visit · open** or **Visit · signed**. They are started with **Start visit** and signed as described in [Visits and notes](/docs/en/guides/visits). The **Add** menu has no visit entry. To add a file, choose **Add → Upload file**. If the same file was uploaded a moment ago, Salisan asks before it keeps a second copy.

## Start from a template [#template]

Open **More actions** in the document and choose **Import template**, then pick a published template and a language.

<Callout type="warn">
  Importing replaces everything written in the document, and undo will not bring the old text back. The earlier version stays in **History**.
</Callout>

Salisan asks you to confirm with **Replace text**. Values that have no data on record stay visible as variables, so check them afterwards. Templates are managed in [Templates](/docs/en/guides/templates).

## Review changes from the Agent [#agent-changes]

Ask the [Agent](/docs/en/guides/agent) from the open document or about selected text. Its proposals appear in the text as changes you review.

* **Keep** accepts a change. **Dismiss** rejects it. **Keep all** and **Dismiss all** act on every change. **Why** shows where a change came from, for example a transcription.
* Keeping text accepts the words only. A proposed diagnosis, medication or other finding stays a draft until you record it. Complete it and choose **Add 2 to the record** (see [Visits and notes](/docs/en/guides/visits#drafts)).
* A proposed appointment is not booked until the booking is confirmed.

If **Restore proposal** appears after an interruption, restore the saved proposal and review it before you ask again. You stay responsible for everything you keep.

## Download, archive or withdraw [#withdraw]

Use the row menu in **Documents & files**, or **More actions** inside an open document.

* **Download PDF** (in **More actions**) saves a copy to your device. It does not send anything to the patient. To email a patient, see [Patient messages](/docs/en/guides/patient-messages).
* **Archive** (row menu) moves a document out of the working set into **Archived**. **Unarchive** brings it back.
* **Discard** (row menu) deletes a document that is still blank. Nothing was recorded in it.
* **Withdraw…** takes back a document written for the wrong patient or added in error.

Withdrawing lists the entries the document captured in the record. Each is ticked, so they are withdrawn with it. Untick any you want to keep, then give a reason and choose **Withdraw document** (or **Withdraw document and 2 records**). An entry also captured in another document is marked.

<Callout type="warn">
  A withdrawn document stays readable and is marked **Withdrawn**. It is not deleted. Entries you withdraw with it are struck through in the record. Choose **Undo withdrawal** if you withdrew it by mistake. If the patient's record is under a legal hold, withdrawal is refused.
</Callout>

An uploaded file has its own withdrawal, with the reason **Uploaded in error**.
