# Recording and transcription (/docs/en/guides/transcription)

A transcription belongs to the note or care plan background where you record it. The transcript and the audio are kept separately, and the audio can be deleted while the transcript stays.

## Record in a note [#record]

You need write access to the patient's clinical records and permission to use AI features. Your practice must have transcription enabled. A patient who is excluded from AI assistance cannot be recorded or transcribed.

1. Open the visit, document or care plan background.
2. Type `/` and choose **Transcribe audio**.
3. The first time, read **Before you record** and choose **I understand**. Allow the microphone in your browser.
4. Check that the block says **Recording**. Use **Pause** and **Resume** when needed.
5. Choose **Stop**. Wait until the status changes from **Processing** to **Ready**, then open **Full transcript**.

<Callout type="info">
  Telling the patient about the recording, and getting consent where the law requires it, is your practice's responsibility. Salisan does not ask patients for you.
</Callout>

Only one recording can run in a document at a time. If your microphone is unavailable, open the transcription page and record there. That page also lets you **Upload audio**. Each take becomes its own chapter of the same session, so check every take rather than assuming they all worked.

## Check the transcript [#review]

Play the audio and compare the words, numbers and speaker turns that matter. Choose **Play from here** on a line to hear it.

* Speakers are detected automatically and names are never guessed. Use **Rename speaker** to label them.
* A take marked **Failed** was saved but not transcribed. Choose **Retry transcription**. Other takes are not affected.
* **Export transcript** and **Download audio** save copies to your device.

A transcript is raw material. Correct it before you rely on it.

## Write up the findings [#write-up]

When recordings are ready, choose **Write up** in the note. It shows how many recordings it will use. The Agent adds text and draft findings to the note, each linked to the moment in the recording it came from. It also tells you how many items it was unsure about.

Review each change with **Keep** or **Dismiss**, correct the draft values, and record the findings you want. Writing up does not sign the note, send a referral or book an appointment. See [Visits and notes](/docs/en/guides/visits#drafts) and [Patient documents](/docs/en/guides/patient-documents#agent-changes).

## What happens to the audio [#audio]

By default, audio is kept with the patient's record. Only a practice owner or admin can change this, under **Recordings** on the **Chats** tab of [Agent settings](/docs/en/guides/agent-settings):

* **Keep with the patient record**: no automatic deletion.
* **Delete after 30 days**: the audio is deleted 30 days after the transcript or speaker labels were last saved.
* **Delete once the transcript is final**: the audio is deleted once speaker matching has finished, within 72 hours at most.

<Callout type="warn">
  Deleting audio cannot be undone. It removes every stored copy. The transcript stays readable, but you can no longer play the recording back or transcribe it again. The practice setting applies to recordings that are already stored.
</Callout>

Audio from a failed or abandoned take is deleted after its own period under every setting. A legal hold on the patient keeps the audio until it is released. A file that was already a patient document is detached from the transcript, not deleted.

When the audio is gone, the recording block in the note says it was deleted. Tell patients how long your practice keeps recordings.
