# Record a consultation and write it up (/docs/en/learn/clinician-dictation)

After this lesson you can record inside a note, check the transcript and review the text and draft findings that come from it. It takes about 15 minutes. You need a microphone and write access to the patient's clinical records.

## Why it matters [#why]

Recording lets you talk to the patient instead of typing. The recording is raw material: a transcript can mishear a number or a name, and the text written from it is a proposal. Your review is what turns it into documentation.

Recording is also something you tell the patient about. Salisan does not ask patients for consent. Telling them, and getting consent where the law requires it, is your practice's job.

## How it works [#how]

Anna de Vries agrees to the recording. Her doctor opens the visit note, types `/` and chooses **Transcribe audio**.

### Before the first recording [#before-the-first-recording]

The first time you record, a notice titled **Before you record** appears. Read it and choose **I understand**. Then allow the microphone in your browser. You confirm the notice once.

You can record when these are true: your role has the **Use AI features** permission, which every default role has, your practice has transcription enabled, and the patient is not excluded from AI assistance. Patients under 18 are excluded automatically. If the patient is excluded, the block says so and its **Record** button stays off. If transcription is not enabled for the practice, Salisan tells you to ask an administrator. Only one recording can run in a note at a time.

### Record and check [#record-and-check]

The block shows **Recording** with a timer, **Pause** or **Resume**, and **Stop**. After you choose **Stop**, the block shows &#x2A;*Processing…** and then a player. Choose **Full transcript** to read it. Play the audio and compare the numbers, names and speaker turns that matter. Speakers are detected automatically and never named for you. Use **Rename speaker** to label them.

If a take fails, the audio is kept. Choose **Retry transcription**. Other takes are not affected.

### Write up [#write-up]

When a recording is ready, the button **Write up · 1** appears at the bottom of the note. The number is how many recordings it will use. Choose it. The Agent adds text to the note and draft findings, each linked to the moment in the recording it came from. It also tells you how many items it was unsure about, for example **1 unsure**.

You review every change with **Keep** or **Dismiss**. A kept finding is still a draft. It reaches the record only when you add it with **Add 1 to the record** or sign the note, as in the previous lesson. Writing up never signs the note, sends a letter or books an appointment.

### The audio afterwards [#the-audio-afterwards]

By default the audio stays with the patient's record. An owner or admin can change this under **Agent settings**, on the **Chats** tab, under **Recordings**. They can delete audio after 30 days or once the transcript is final. The transcript stays when the audio is deleted.

## Try it [#try-it]

Use your own voice. Do not record a real patient for an exercise.

1. Open **Sam Testpatient**. Check in **Patient actions → Edit** that **Exclude from AI assistance** is not ticked.
2. Choose **Start visit**.
3. Type `/` and choose **Transcribe audio**. If the notice **Before you record** appears, choose **I understand** and allow the microphone.
4. Say a few sentences, for example: "Blood pressure one twenty-eight over eighty-two. Heart rate seventy-two. No headaches this week." Use **Pause** and **Resume** once, then choose **Stop**.
5. Wait for &#x2A;*Processing…** to finish. Choose **Full transcript** and compare it with what you said.
6. Choose **Write up · 1**. Wait until the line below the note reads that it was written from 1 recording.
7. Review each change. Check the values against the transcript. For this exercise, choose **Dismiss** on each change, or **Dismiss all** if a strip offers it.
8. Sign the visit if you are done, or leave it open for the next lesson.

If you see a message that transcription is not enabled, or that credits are used up, ask an administrator. Both are practice settings.

## Check yourself [#check]

1. Who tells the patient about the recording?

<details>
  <summary>
    Answer
  </summary>

  Your practice does. Salisan does not ask patients for consent. The notice before the first recording says so.
</details>

2. After **Write up**, the note contains a new diagnosis block. Is the diagnosis in the record?

<details>
  <summary>
    Answer
  </summary>

  No. It is a draft. Check it against the transcript, then add it with **Add 1 to the record** or sign the note.
</details>

3. What happens to the audio by default, and who can change that?

<details>
  <summary>
    Answer
  </summary>

  It stays with the patient's record. An owner or admin can change it in **Agent settings** under **Recordings**. The transcript stays if the audio is deleted.
