Publish a form for your team
Create a form in the Library, publish it so staff can send it and archive content you no longer use.
After this lesson you can create a form, publish it so staff can send it to patients, and archive it again. It takes about 20 minutes. Creating and publishing needs the permission to manage templates, which Owner and Admin have. Every default role can use published templates.
Why it matters
Patients only receive what you have published. A saved draft is not offered to staff when they send a request or write an email. Publishing is the step that makes your work reach people, so it is worth knowing exactly what happens before and after it.
How it works
Fenna, the practice manager, writes a pre-visit questionnaire for Dr. Visser. It has a single-choice question for the reason for the visit and a long answer for anything the patient wants to discuss.
The Library
Open Library, under Manage in the sidebar. It holds your Forms, Reports, Emails and Skills, including content added from the Marketplace. Search with Search your content… or pick a kind to narrow the list. A card shows Draft ahead of published when you have edits that patients do not see yet.
Create and publish a form
Choose Create template, then Form. Enter a title and an optional description. The Blocks panel offers question types, for example Text input, Long text input, Single choice, Multiple choice, Date input and Vitals. Click a block to add it.
Write each question in its label, where the placeholder reads Add a question. Add Helper text if the patient needs it. Turn on Required when an answer must be present. Switch to the preview and read the form the way the patient will see it.
When saving and any translation have finished, choose Publish. While changes are still saving, or the form is empty, Publish tells you to wait or to add content. After publishing, staff find the form in the Send to patient dialog on the patient's Requests page.
Editing a published form does not change what patients receive until you publish again. The published version stays available while you work.
Ready-made content
Choose Add from the Marketplace… to find forms, reports, emails and skills from publishers. Open a listing, read its publisher, languages and license, and choose Install. Installing needs the permission to activate content, which Owner and Admin have. Check the content before you rely on it.
Archive what you no longer use
On a card, open More actions and choose Archive…. Archived content is hidden from active use. Existing work and imported copies are kept, and you can restore it later. A form with active schedules must have them paused or replaced first.
Try it
A published test form is visible to every colleague who sends requests, so name it clearly and archive it when you finish.
- Open Library, choose Create template, then Form.
- Title it "TEST intake (do not use)". Add a Long text input, write the question "What would you like to discuss?" and turn on Required.
- Open the preview. Check that the question and the required marker look right.
- Wait for the save indicator to read saved, then choose Publish.
- Open Sam Testpatient, choose Requests and then Send to patient. Find your form in the list. Close the dialog without sending, or send it to your own address as in the front desk course.
- Return to Library. On the form's card choose More actions → Archive…, then Archive.
- Open Send to patient again. The form can no longer be sent.
Check yourself
- You saved a form, but staff cannot pick it when they send a request. Why?
Answer
It is not published. Saved drafts are not offered. Choose Publish once saving and translation have finished.
- You change a published form and have not published again. What do patients receive?
Answer
The published version. Your changes stay in the draft, marked Draft ahead of published, until you publish.
- What does archiving do to answers patients already gave?
Answer
Nothing. Archiving hides the form from active use. Existing results are kept, and you can restore the form later.
